Survey for Health Care Payment Reform Project

Dear Colleagues,

MCNP is representing you in two big projects that will help shape heath care reform in Massachusetts. In order to assure our inclusion in the medical home demonstration projects and in payment reform discussion we need to collect some background information. I am asking you to take a few minutes to fill out a survey that will provide me with current information about your practice. The survey is brief and should take only 3 to 5 minutes of your time. This information will be confidential. I will only share data, not personal information. The data will be used to develop our political strategy to make sure we are fairly represented in all of the health reform discussions.

I want to thank you in advance for your assistance and I will also post the results of this survey on the MCNP website for your review. If you have any questions or concerns, please contact me.
